following a section 8 notice help..

hi all,
At the beginning of the month I received a noise abatement notice from the council concerning noise nuisance from a flat I rent.
On Sunday just gone (14/7/13) I served a section 8 notice claiming ground 14 informing that court proceedings wouild not begin until 15/7/13, this was posted through the letterbox as the tenant was not in and witnessed by my wife (first question, is she a valid witness?).

Late Sunday night and into Monday I receive a number of texts from the tenant (he's polish and his English is not the best) but from what I can understand from his texts he believes he has 2 months to stay and he was also a bit threatening.

Now I need to complete the legal forms N005 & N119 I believe and submit them to the local court and I have a few questions..

1. The tenancy was in joint names (him and his girlfriend) but she moved out after 2 months. I served the section 8 to both names (as on the AST) but do I need to do the same with the N005 & N119?

2. Do I need to include copies of the section 8 notice and the noise abatement notice sent to me by the council?

3. How many copies of everything do I need to send to the court?

4. Do I need to include a witness statement concerning the delivery of the section 8, and if so (as mentioned above) is it OK for my wife to be the witness?

5. On N119, what's question 9 (a & b) about forfeiture all about? Do I need to complete those details?

As always, thanks in advance for any help offered..
